Incident Report Request
The Aurora Fire Rescue is pleased to offer Fire, Motor Vehicle Accidents, and Patient/Medical incident reporting that occur within the City of Aurora.
If the request is for a patient care report for you, you must provide:
- A valid driver's license or ID (you must come to Fire HQ to make a copy)
If the report requested is about someone else you must provide:
- A valid driver's license or ID (you must come to Fire HQ to make a copy) and
- A signed, notarized letter stating release of information, or legal guardianship.
If the report is for a third party (insurance company, attorney or private investigator), please request records through the City Clerk's Office.
